For diagnostic tests, the diagnosis is the health condition that the test is intended to identify.

In studies of treatment effects, a diagnosis can be used as an eligibility criterion (e.g. patients with a diagnosis of acute heart attack or stroke in a study of treatments for those conditions), or an outcome (e.g. patients experiencing an acute heart attack or stroke in a study of treatments to prevent those outcomes).
